Supplemental Event Reporting:  A Proactive Framework to Promote Patient Safety and Prevent Avoidable Losses

As part of CHRMS' mission to provide the membership with topical risk management-related information, the first of the webinar lecture series for 2011 -2012 will be presented on Thursday, October 6, 2011 from 12:00 p.m. - 1:00 p.m.

We are please to announce our speakers for this session are:

• Stephen Pavkovic, RN, MPH, JD, Risk Manager, Northwestern Memorial HealthCare

Stephen Pavkovic brings a diverse background to his current role as a Risk Manager at Northwestern Memorial Hospital.  While working as an operating room nurse and a manager he earned advanced degrees in public health and law. He then practiced law as an associate for a Chicago firm defending medical malpractice claims and as a State’s Attorney for the Cook County Bureau of Health Services.  At Northwestern, he applies active risk management strategies to identify patient safety opportunities and to control avoidable losses.  He remains professionally active with several national publications and presentations. 

• Stacey A. Cischke, Partner, Cassiday Schade, LLP

Stacey Cischke is a partner with Cassiday Schade LLP. Ms. Cischke concentrates her practice on the representation of physicians, dentists and healthcare institutions. She represents several Chicago area teaching hospitals and physician groups in medical negligence cases. Ms. Cischke handles all aspects of litigation and has tried a number of medical malpractice cases to verdict.

In addition to her litigation work, Ms. Cischke provides pre-litigation counsel to the firm’s healthcare clients to improve understanding and prepare them for responding to discovery during litigation. She also speaks to physician and administrator groups focusing on topics such as understanding the defense of birth injury cases, preparing for depositions and improving reliability in responding to discovery from an institutional standpoint. Ms. Cischke is an Adjunct Professor of Law in the Online Legal Education Beazley Institute for Health Law and Policy, Loyola University Chicago School of Law teaching "Legal Issues in eHealth". In 2010, she co-authored a chapter entitled, “Principles for Strategic Discovery” in the book, Principles of Risk Management and Patient Safety.

Ms. Cischke is a graduate of James Madison College at Michigan State University where she graduated with honors and a member of Phi Beta Kappa. During law school, Ms. Cischke was a member of the Thomas Tang Moot Court team and was awarded the honor of Best Brief.

Ms. Cischke serves as a member of the firm’s Recruitment and Diversity Committees and is an alumni member of the Dean’s Diversity Council for Loyola University Chicago School of Law.
She is admitted to practice in Illinois and the U.S. District Court for Northern District of Illinois.

Program: 

Frequently, risk management departments depend on voluntary reporting to learn about adverse events.  Yet, experienced risk managers and patient safety advocates know that voluntary adverse event reporting provides only a partial snapshot of actual patient risk.   

For example, consider a laparoscopic case emergently converted to open for concern of a trocar-related bowel injury.  Rather than reporting the underlying potential patient harm resulting in an emergent escalation in care, the voluntary adverse event report only captures an inaccurate instrument count event. 

Supplementing event reporting can provide proactive organizations with insight into unreported or misreported events.  However, without a strategic focus the supplemented material can quickly create an overwhelming “drink of water from a fire hose” phenomenon. 

This presentation will provide insight into the prevalence of event underreporting and discuss how often readily available health information can supplement voluntary reporting to identify unknown loss control opportunities. 

The objectives of this presentation are to identify and discuss the benefits of:

• supplementing existing event reporting mechanisms with proactive data collection;
• utilizing Enterprise Risk Management-based tactics to support proactive data collection activities; and
• addressing discovery concerns before initiating proactive event identification.
